Completing and
submitting this Enrolment Form will secure one 'tentative' place in
class on the date you have selected above. If you are booking
in more than one student, an on-line enrolment needs to be submitted
for each individual student.
An invoice will be
generated and emailed to you for payment (invoices will be posted
when an email address is not supplied).
Full payment needs
to be received no later than 7 days prior to your course date, which
will secure your place/s in the class. If full payment is not
received within this time, your place/s in the class may be cancelled.
Vital Training
Solutions accepts payment by Credit Card (Visa or MasterCard), BPAY,
PostBillPay, Direct Deposit or Cheque
(provided there is enough time for cheque clearance prior to course
commencement).
Cancellation from any course needs to be received
in writing no later than 48 hours prior to the course commencing, in
which you may choose to enrol on a different date, or in a different
course, or be refunded in full (excluding any processing fees).
If the required notice is not given, you will
forfeit any fees paid to Vital Training Solutions. I have read, understand
